Does Usability Trump Accessibility?

I know I am going to get flack for this, but WTF is the difference between accessibility and usability? I still am befuddled why this “dichotomy” exists.
One way to build something that is usable is to understand the people who will be using it. Knowing user characteristics is important so you know how to adapt your design based on those characteristics.
Why do they have to live in harmony if they are the same thing?
