Internet bank records wiped after upgrade fiasco -- "Lloyds TSB has accidentally wiped the records of 1.6 million online customers during a computer upgrade." (Comments: This problem illustrates the biggest risk associated with Customer Relationship Management. While you are able to give customers what they want when they want it, you also risk showing customers your mistakes. Sometimes you can get away with making mistakes, and sometimes you can't. In this case, the error is huge and it is hard to ignore.)
